Simultaneous Recession of 4 Rectus Muscles and Cryotherapy for Neovascular Glaucoma and Absolute Glaucoma


Abstract
The treatment of eyes with neovascular glaucoma and absolute glaucoma is often less than satisfactory. The simultaneous recession of 4 rectus muscles and cryotherapy was performed in 8 eyes with neovascular or absolute glaucoma.
The purpose of surgery was to destroy all anterior ciliary arteries from the rectus muscles and to produce anterior segment ischemia. We applied cryotherapy on the sclera between the original insertion site and the new insertion site of the
rectus
muscles. The cases were followed for periods ranging from 8 to 28 months. At the end of follow-up periods, we observed a statistically very significant drop(P<0.01) in the intraocular pressure level decreasing from the preoperative level of
47.12¡¾12.3mmHg(mean¡¾S.D) to 31.5¡¾13.23mmHg without any additional medical therapy and our patients showed decrease or loss of eyeball pain corneal edema and iris neovascularization.
Clincally, the simultaneous recession of 4 rectus muscles and crytherapy is considered as one of the good methods of treatment of absolute and neovascular glaucoma.
